If you have booked into any session at Rockburn, or have booked in children whom require an instrutor to lead/supervise the session, you will need to complete an Instructed Session Form PRIOR to arriving for the session.
If you are already on our system, having climbed here before, you are still required to complete an Instructed Session Form PRIOR to arriving for any session that requires you to receive instruction.
This is part of our duty of care: Rockburn Ltd is required to have a log and record of your attendance and confirmation that you have read and understood the BMC Participation Statement, Conditions of Use and Rules of Rockburn for EACH session in which you receive instruction on a formal, paid basis.
You can complete the online form below and submit it online. We can provide paper/online copies onsite, however, this will increase you waiting time prior to the session. If you do not have access to a printer, please arrive 15 minutes prior to your session to allow time to complete the necessary forms prior to the start of the session.
